Behavioral Health Beyond the Therapy Room.

Person-centered support for individuals, older adults, caregivers, and the communities that serve them.

Priority Path Therapy Group provides behavioral health support for older adults navigating changes in health, independence, or living arrangements, as well as families adjusting to a loved one's changing needs. We support residents transitioning to or living in assisted living communities and nursing homes, while helping family members and caregivers navigate stress, grief, emotional challenges, and care transitions. We also welcome partnerships with senior living communities, skilled nursing facilities, and healthcare organizations to strengthen support for residents, their families, and care teams.

Support That Meets You Where You Are

  • Individual & Family Support

Life changes can affect every aspect of our well-being, including our relationships, sense of independence, and emotional health. Whether you're coping with grief, adjusting to changes in health, managing caregiver stress, or supporting a loved one through a difficult transition, behavioral health services provide a supportive space to explore challenges, identify resources, and develop practical strategies for moving forward. Our approach recognizes the needs of both individuals and families, helping them navigate change with greater understanding and support.

  • Assisted Living & LTC Facility Support

Transitioning into an assisted living community or nursing home can bring significant emotional and social changes for residents and their families. Behavioral health support can help individuals adjust to new environments, cope with changes in independence, process grief and loss, and maintain a sense of dignity and connection. We work to support the emotional and psychosocial needs of residents while helping families navigate care transitions and identify resources that contribute to overall well-being and quality of life.

  • Behavioral Health Partnerships

Priority Path Therapy Group collaborates with assisted living communities, skilled nursing facilities, rehabilitation settings, and other healthcare organizations to support the behavioral health needs of the individuals and families they serve. Partnership opportunities may include resident adjustment support, grief and life-transition groups, wellness-focused programming, family and caregiver guidance, and resource coordination. Through a person-centered approach and collaboration with care teams, we aim to strengthen support systems and promote emotional wellness within care communities.
